PASSPORT AND VISA
 
 
U.S. Passport
 
If you want to travel overseas for surgery/treatment, you will need a valid U.S. passport. A passport is an official document issued by a government, which identifies the holder and grants him or her permission to travel abroad.  The U.S Department of State only has the authority to grant, issue and verify U.S. passports.
 
Necessary steps to obtain a Passport
 
1. To obtain a passport for the first time, the applicant must go in person to one of the 7,000 passport acceptance outlets located throughout the United States.
 
2. The applicant must carry two passport-size photographs, proof of citizenship, and valid photo identification (such as a driving license).
 
Acceptance outlets include Federal, State and Probate courts, Post Offices and a number of county and municipal offices. If the passport is required urgently, there are 13 regional passport agencies who can process in an expedited manner, prior appointments are required in such cases. Visa to India
 
You should have a valid visa to travel to India for your medical treatment. The Indian Embassy in the U.S provides you with necessary information and services about how obtain a visa. For more information, please visit

The following documents are needed for a Visa application:
 
• A valid passport with minimum six months validity from the date of application along with two full blank pages.
 
• Two recent passport size photographs (black/white or color) with a front pose against a light background.
 
• Supporting documents should be submitted if the application is other than a tourist visa.
 
Visa Fees
 
Generally, for a 6-months Visa, a U.S. Citizen has to pay $60.00 as Consular Fee along with a Service Fee of $13.00.
 
Persons of Indian Origin (PIO)
 
Persons of Indian Origin applying for Visa on U.S. passport for the first time must turn in the Indian passport for cancellation including a renunciation form.
